Once you’ve completed all the required steps, you’ll be placed on our priority scheduling list. We'll contact you to schedule your child's evaluation as soon as openings are available.

A current referral from your child's primary care provider is required to schedule.

• If your referral is more than one year old, please request an updated one.

💻 Submit Your Intake Forms:
If you haven't already, please complete your intake paperwork by clicking the "Intake Forms" button on our website.

• Not sure if you've submitted your forms? Email [email protected] to confirm.

⚠️ Important:
If we do not receive your completed intake forms or an email confirming your continued interest within two weeks, we will assume services are no longer needed and your child will be removed from the waiting list.
