Connect the Dots Pediatric Therapy values your privacy. Our website does not collect any personally identifying information without your consent (i.e. submitting information through our contact form). We do not store any data on the site.

All of our patient information is covered by the policies set forth in the Health Insurance Portability and Accountability Act of 1996. You may receive a copy of our HIPPA policies from the front desk. You will receive a copy prior to your first visit as well.

At times, we will share and exchange information with other healthcare providers. For this to occur, you will need to sign the Authorization for the Exchange of Information form. This form will be supplied in the Intake Packet.
