You've come to the right place!
Whether you are a current patient looking to expand therapy services or getting started for the first time, we are excited that you thought of us!
Once a referral has been ordered, a guardian must complete the intake form to proceed with scheduling.
If your child is a NEW PATIENT or has NOT RECEIVED SERVICES IN THE LAST YEAR, you will need to complete the NEW PATIENT INTAKE FORM.Â
To proceed with your child's referral, please complete intake form. Once your intake form has been submitted, our office will call you within 3 weeks to discuss scheduling.
