Before you begin the intake form, here’s what you need to know:
- Only fully submitted Intake Forms will be added to the waitlist or move forward with scheduling, depending on therapy availability. Once submitted, a PDF copy of the completed form will be available to download.
- Plan to spend 20–30 minutes completing this form. If you need to pause - save your progress, as unsaved entries will be lost.
- Completion of the form does not guarantee services. Our office will reach out if your concerns fall outside our scope of care.
- Late afternoon appointments are currently unavailable. We have scheduling availability only for morning and early afternoon time slots.
- Due to various factors such as staffing availability and transitions as children graduate from services, we are unable to provide specific timelines for our waiting list. We truly appreciate your understanding and assure you that we are working diligently to support your family.
