We're excited to let you know that your child is coming up on our waiting list to schedule for services!

To proceed, please CLICK the "Intake Forms" button to complete the appropriate intake paperwork as soon as possible.

• If we do not receive the intake form within two weeks, we will assume services are no longer needed and remove your child from our waiting list.
• Currently we are scheduling evaluations 1-3 months in advance, and at this time we only have morning and midday
  appointments available for treatment.

AN ACTIVE REFERRAL IS REQUIRED TO SCHEDULE:

• Please note that we are required to have a current referral on file to proceed with scheduling.
  • If your referral is more than one year old, please contact your primary care provider to request a new one. After submitting the intake form, you will receive a notification if your referral has expired.
